Application for changing department for Enrolled students, fall 2015

 

This is to notify you of information about application for changing department for enrolled students in Fall semester, 2015.Those who want to change the department have to submit the application form within the designated period below.

 

1. Qualification for Application

 a. Enrolled students who completed one or two semesters at the said graduate school

 b. For any of the degree courses (Master’s, Ph.D and Integrated courses)

 

2. Required Documents

a. A copy of the Application form for changing department (Prescribed form) : Please refer to Attachment #1

b. Application for Credit Recognition from the change of department : Please refer to Attachment #2

 

3. Place for submission : Department office for moving in (Department office that you want to join)

 

4. Application period for changing department : Until Jul.10th(Fri), 2015

 

5. Notes

a. The number of application for changing department : Only once while in school.

b. Confirmation of changing department and reflection of school register will be processed from Aug.31st(Mon), 2015.
